What is Case Management?
The Case Management Society of America (CMSA) defines case management as a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individuals needs through communication and available resources to promote quality cost-effective outcomes.
Case Management is a human service provided by professionals from diverse academic preparations and backgrounds. These professionals are trained in and licensed to perform assessments in their respective field.
Case Management is intended to assist the client to access needed care & services on a timely basis and to prevent inappropriate institutionalization through a thorough consideration of community-based alternatives.
